Compensation Range: $60,000 - $65,000 yearly commensurate with experience

Overview:
The Housing Works Emergency & Transitional Housing Program (ETH) provides time limited transitional housing and support services to individuals reentering the community following periods of incarceration or other justice system involvement. Program participants engage in a variety of onsite services, including case management, vocational training, housing advocacy and mental health support that address barriers to accessing housing in the community and help build skills that participants can use to achieve and maintain stable functioning in the community. This position will be based at the Fresh Meadows, Queens program site. This program location is designed for participants with higher level of care needs including mental illness, substance use disorders or chronic medical conditions. Reporting to a Case Management Supervisor, the Housing Case Manager will be primarily responsible for conducting psychosocial assessments and intake procedures with newly enrolled clients, co-developing robust interdisciplinary treatment plans, connecting individuals with the services to address social determinants of health, facilitate group level interventions focused on developing and enhancing skills for independent living and assisting participants with identifying and transitioning to more permanent housing alternatives. This is an ideal opportunity for professionals with experience or passion to work within the reentry or housing services sector seeking to further develop skills in the area of clinical case management and work alongside an interdisciplinary team of like-minded professionals committed to improving outcomes for justice-involved individuals.
